Icon Vitality Clinic Policy At Icon Vitality, we are committed to providing exceptional care and creating a welcoming and safe environment for all our patients. To ensure the highest standards of service and professionalism, we have established the following clinic policy:

1. Patient Confidentiality: We respect your privacy and adhere to strict confidentiality guidelines. Your personal and medical information will be kept confidential and will not be disclosed without your consent, except as required by law.

2. Appointment Scheduling: To best serve our patients, we operate on an appointment-based system. We kindly ask that you schedule appointments in advance to minimize wait times and ensure sufficient time for your consultation or treatment.

3. Cancellation Policy: We understand that unforeseen circumstances may arise, and you may need to reschedule or cancel your appointment. We kindly request at least 24 hours' notice for appointment cancellations or changes. Failure to provide sufficient notice may result in a $50 cancellation fee, which will be charged to the credit/debit card on file.

4. Payment Policy: Payment is due at the time of service. We do not accept insurance at this time. We accept cash, credit/debit cards, and other forms of electronic payment. For your convenience, we can provide detailed invoices for your records.

5. All Services Are Final: All services provided by Icon Vitality are final. Once a service has been rendered, no refunds or exchanges will be provided unless otherwise specified.

6. Patient Responsibilities: We believe in collaborative care and encourage our patients to actively participate in their treatment plans. We ask that you provide accurate and complete information about your medical history, follow your prescribed treatment plan, and communicate any concerns or changes in your health status to your healthcare provider.

7. Discharge Policy: Icon Vitality reserves the right to discharge patients for failure to pay outstanding balances, disputed credit card transactions, failure to follow prescribed treatment plans, or behavior deemed inappropriate or disruptive to the clinic environment.

8. Respectful Conduct: We are committed to maintaining a respectful and inclusive environment for all patients and staff. We expect all individuals to conduct themselves in a courteous and respectful manner towards others at all times.

9. Feedback and Concerns: Your feedback is valuable to us as we continually strive to improve our services. If you have any questions, concerns, or suggestions, please do not hesitate to reach out to our clinic administration.
