Affordable, Personalized Direct Primary Care for You and Your Family
Direct Primary Care (DPC) offers patients affordable, transparent healthcare by eliminating the middleman. With a flat monthly membership fee, patients receive unlimited access to their doctor 7 days from 8:00am - 6:00pm without the hassle of insurance. Enrollment fee for each patient is added to first appointment, $249, with a monthly fee of $99/month. It is due on the same day every month and requires a 12 month commitment. We do offer home visits for a fuel charge based on distance.
A La Carte
All new patients will be charged a one time enrollment fee.
Adult at Home Visit - $199
Up to 45 minutes, at home visit. Ideal for those who do not wish to wait in the waiting room, only to be seen for 10 minutes by their provider. You are treated in the comfort of your home. Ideal for sick visits, chronic disease management, medication refills, lab ordering, and imaging ordering. *Additional fuel charge applies.
Additional Add on Patient - $100
Telemedicine - $129
Telemedicine Visit Family add on - $55
Sports physicals - $125
Completion of forms/paperwork - $50
